Aliens: Fireteam Elite 2 uses five named difficulty tiers: Casual, Standard, Intense, Extreme, and Insane. Older preview articles that say Rookie or Veteran are talking about a different ladder — ignore them on this wiki. This page explains what each tier changes, how Insane unlocks after the 25 August 2026 launch, and how to survive once friendly fire and ammo crates start deciding runs. Insane is not selectable on a fresh save. Clear Prologue through Final Endeavor on Casual, Standard, Intense, or Extreme first. Then Insane appears in the difficulty list for campaign and Horde. Mission order: Campaign walkthrough.

What actually scales

Across the ladder you will feel:

  • Enemy health and damage climb.
  • Spawn density and flank chance rise — ceiling runners and rear packs punish tunnel vision.
  • Special elite pressure arrives earlier (Warden, Siren, Praetorian, Pathogen Harbinger, synth Bulwark).
  • Ammo economy tightens. Crates have fewer charges on higher tiers; sidearms with unlimited ammo become emergency tools — Sidearms.
  • Friendly fire matters more. Rockets and explosives wipe pubs on Intense+.
  • Synthetic companions revive less often on Extreme/Insane before a checkpoint forces a reset.

Elemental rules stay the same: thermal melts Xenomorphs; electric punishes synthetics. Bring both when the briefing shows mixed factions — Enemy Dossier and Ammo & Melee.

  1. First night: Casual Prologue with Machinist or DuelistBest Beginner Class.
  2. Story clear: Standard campaign for an honest first ending.
  3. Farm: Intense missions for Hidden Caches and clear rewards — Unlock Weapons.
  4. Horde unlock: Finish campaign, talk to Endeavor officers — Unlock Horde.
  5. Insane prep: Extreme with a human Medic + Machinist before you toggle Insane.

Do not stack four lethal Challenge Cards on your first Intense night. Cards multiply requisition and risk; on Insane they are how organised squads wipe in wave two.

How to beat Insane

Squad shape. Medic for stims and stations, Machinist for turrets, Marauder or Duelist for clear, fourth seat flex (Hunter control or Specialist hybrid). Use the Squad Planner. Solo Insane is possible with bots but brutal — see How to Play Solo.

Positioning. Hold one or two chokepoints. Look up. Call flanks. Do not split for distant objectives while a Praetorian is live.

Ammo. Sidearm stragglers. Primary and signature for elites. Top crates before leaving a room. Patch 1 closed infinite first-aid exploits — play fair, farm clean.

Abilities on cooldown, not panic. Empty Q buttons before the swarm crest, not after three marines are down.

Voice. Insane without callouts is gambling. Crossplay covers Steam, Epic, PS5, and Xbox with region matchmaking after Patch 1.

Horde difficulties

Horde uses the same Casual–Insane names. Unlock the mode first, then climb. Five-wave rounds with a shop between rounds; faction can roll Xenomorphs, Pathogen, or Weyland-Yutani synthetics — Horde Maps. Extract greed on Insane Horde ends runs.

Common mistakes

  • Jumping to Intense before you know crate spawns.
  • Treating Insane as a cosmetic badge instead of a gear and voice check.
  • Bringing only thermal into a synth-heavy briefing.
  • Solo Insane with four Challenge Cards and no Medic bot.
  • Copying pre-Patch 1 perk numbers — Hard to Hit, Explosive Rounds, and companion perks all changed.

How do I unlock Insane?

Finish the full campaign on Casual, Standard, Intense, or Extreme. Insane then appears in the difficulty list for campaign and Horde.

Is Insane available on day one saves?

The tier exists at launch, but your save must clear the story on another difficulty first. Fresh profiles cannot select it.

Should I play Insane solo?

Only after Extreme feels comfortable with bots. Revive limits and ammo crates make solo Insane much harder than Standard.

Do Challenge Cards work on Insane?

Yes. All four marines can stack cards. On Insane, avoid stacking conflicting lethal debuffs unless the squad agrees.

Did Patch 1 change difficulties?

Patch 1 rebalanced perks and closed exploits; it did not rename the Casual–Insane ladder. Rebuild explosive Duelist boards before you copy old guides.
